ABSTRACT

Introducción. Con el uso de la nutrición parenteral agresiva en recién nacidos de muy bajo peso, se detectaron alteraciones del metabolismo fosfocálcico. En 2016 se implementó una estrategia de prevención a través del monitoreo fosfocálcico y su suplementación temprana. El objetivo fue estudiar si esta estrategia disminuye la prevalencia de osteopenia e identificar factores de riesgo asociados. Población y métodos. Estudio cuasiexperimental que comparó la prevalencia de osteopenia entre dos grupos: uno después de implementar la estrategia de monitoreo y suplementación fosfocálcica (01/01/2017-31/12/2019), y otro previo a dicha intervención (01/01/2013-31/12/2015). Resultados. Se incluyeron 226 pacientes: 133 pertenecen al período preintervención y 93 al posintervención. La prevalencia de osteopenia global fue del 26,1 % (IC95% 20,5-32,3) y disminuyó del 29,3 % (IC95% 21,7-37,8) en el período preintervención al 21,5 % (IC95% 13,6-31,2) en el posintervención, sin significancia estadística (p = 0,19). En el análisis multivariado, el puntaje NEOCOSUR de riesgo de muerte al nacer, recibir corticoides posnatales y el período de intervención se asociaron de manera independiente a osteopenia. Haber nacido luego de la intervención disminuyó un 71 % la probabilidad de presentar fosfatasa alcalina >500 UI/L independientemente de las restantes variables incluidas en el modelo. Conclusión. La monitorización y suplementación fosfocálcica precoz constituye un factor protector para el desarrollo de osteopenia en recién nacidos con muy bajo peso al nacer.


Introduction. With the use of aggressive parenteral nutrition in very low birth weight infants, alterations in calcium and phosphate metabolism were detected. In 2016, a prevention strategy was implemented through calcium phosphate monitoring and early supplementation. Our objective was to study whether this strategy reduces the prevalence of osteopenia and to identify associated risk factors. Population and methods. Quasi-experiment comparing the prevalence of osteopenia between two groups: one after implementing the calcium phosphate monitoring and supplementation strategy (01/01/2017­12/31/2019) and another prior to such intervention (01/01/2013­12/31/2015). Results. A total of 226 patients were included: 133 in the pre-intervention period and 93 in the post-intervention period. The overall prevalence of osteopenia was 26.1% (95% CI: 20.5­32.3) and it was reduced from 29.3% (95% CI: 21.7­37.8) in the pre-intervention period to 21.5% (95% CI: 13.6­31.2) in the post-intervention period, with no statistical significance (p = 0.19). In the multivariate analysis, the NEOCOSUR score for risk of death at birth, use of postnatal corticosteroids, and the intervention period were independently associated with osteopenia. Being born after the intervention reduced the probability of alkaline phosphatase > 500 IU/L by 71%, regardless of the other variables included in the model. Conclusion. Calcium phosphate monitoring and early supplementation is a protective factor against the development of osteopenia in very low birth weight infants.

ABSTRACT

Objective:To study the risk factors for retinopathy of prematurity (ROP) in very low birth weight infants (VLBWIs).Methods:From October 2020 to December 2021, VLBWIs with gestational age(GA) ≤32 weeks admitted to the neonatal department of our hospital were retrospectively studied. According to the occurrence of ROP, they were assigned into ROP group and non-ROP group. The clinical data of the two groups were compared and the risk factors of ROP in VLBWI were analyzed.Results:A total of 251 VLBWIs were enrolled, including 60 cases (23.9%) in ROP group and 191 (76.1%) in non-ROP group. The GA and birth weight (BW) of ROP group were significantly lower than non-ROP group [28(26,30) w vs. 29(28,31) w, 1 035(840,1 285) g vs. 1 260(1 110,1 380) g, respectively, all P<0.001]. The ROP group had longer duration of oxygen therapy [42.0(26.0, 53.5) d vs. 24.0(18.0, 34.0) d], higher incidences of blood transfusion [2.5(1.0, 3.0) times vs. 2.0 (1.0, 2.0) times] and hyperglycemia (80.0% vs. 16.8%), higher average [(5.6±0.5) mmol/L vs. (5.1±0.5) mmol/L] and peak [10.4(7.8,13.2) mmol/L vs. 6.5(6.1,6.8) mmol/L] blood glucose levels in the first week than the non-ROP group (all P<0.001). Multivariate analysis showed that longer duration of oxygen therapy ( OR=1.047, 95% CI 1.008-1.087, P=0.018) and higher peak blood glucose level in the first week ( OR=1.268, 95% CI 1.092-1.474, P=0.002) were the independent risk factors for ROP. Conclusions:Longer duration of oxygen therapy and higher peak of blood glucose level in the first week are risk factors for ROP in VLBWIs.

OBJECTIVES@#To investigate the clinical characteristics and risk factors for early-onset necrotizing enterocolitis (NEC) in preterm infants with very/extremely low birth weight (VLBW/ELBW).@*METHODS@#A retrospective analysis was performed on the medical data of 194 VLBW/ELBW preterm infants with NEC who were admitted to Children's Hospital Affiliated to Zhengzhou University from January 2014 to December 2021. These infants were divided into early-onset group (onset in the first two weeks of life; n=62) and late-onset group (onset two weeks after birth; n=132) based on their onset time. The two groups were compared in terms of perinatal conditions, clinical characteristics, laboratory examination results, and clinical outcomes. Sixty-two non-NEC infants with similar gestational age and birth weight who were hospitalized at the same period as these NEC preterm infants were selected as the control group. The risk factors for the development of early-onset NEC were identified using multivariate logistic regression analysis.@*RESULTS@#Compared with the late-onset group, the early-onset group had significantly higher proportions of infants with 1-minute Apgar score ≤3, stage III NEC, surgical intervention, grade ≥3 intraventricular hemorrhage, apnea, and fever or hypothermia (P<0.05). The multivariate logistic regression analysis showed that feeding intolerance, blood culture-positive early-onset sepsis, severe anemia, and hemodynamically significant patent ductus arteriosus were independent risk factors for the development of early-onset NEC in VLBW/ELBW preterm infants (P<0.05).@*CONCLUSIONS@#VLBW/ELBW preterm infants with early-onset NEC have more severe conditions compared with those with late-onset NEC. Neonates with feeding intolerance, blood culture-positive early-onset sepsis, severe anemia, or hemodynamically significant patent ductus arteriosus have a higher risk of early-onset NEC.

ABSTRACT

El abordaje nutricional en los recién nacidos de muy bajo peso al nacimiento constituye un desafío en la práctica clínica de los neonatólogos, y muchas veces se aborda fuera del período crítico. Existe evidencia contundente de que la optimización nutricional precoz impacta en forma directamente proporcional en la sobrevida y sobrevida sin morbilidades mayores para este grupo. La implementación de lactancia materna precoz en este contexto debe ser una prioridad del equipo asistencial, siendo la mejora de calidad una herramienta de demostrada utilidad para mejorar los resultados en términos de mortalidad y morbilidad neonatal.


The nutritional approach of the very low birth weight infant poses a great challenge to most neonatologists in their clinical practice, and it is frequently delayed until de newborn is in stable clinical conditions. Currently, scientific evidence supports that early nutritional optimization impacts directly on this group's survival and on their survival without major morbidities. Initiatives fostering early breastfeeding should be prioritized by the healthcare team. Quality improvement has shown to be a very useful resource to improve outcomes regarding neonatal mortality and morbidities.


A abordagem nutricional do recém-nascido de muito baixo peso representa um grande desafio para a maioria dos neonatologistas em sua prática clínica, sendo frequentemente postergada até que o recém-nascido esteja em condições clínicas estáveis. Atualmente, evidências científicas sustentam que a otimização nutricional precoce impacta diretamente na sobrevivência desse grupo e na sobrevivência sem maiores morbidades. Iniciativas de incentivo ao aleitamento materno precoce devem ser priorizadas pela equipe de saúde. A melhoria da qualidade tem se mostrado um recurso muito útil para melhorar os desfechos em relação à mortalidade e morbidades neonatais.

ABSTRACT

Objective:To explore the types of venous access, indwelling time and the correlation with catheter-related bloodstream infection (CRBSI) of very and extremely low birth weight infants (VLBW & ELBW) in NICU.Methods:The self-designed venous access data collection form was used to collect the venous access data of VLBW & ELBW infants who were admitted to NICU of Children′s Hospital of Fudan University from January to December 2019. SPSS 22.0 software was used for data analysis, description and binary Logistic regression analysis.Results:A total of 218 cases were collected, including 9 cases of peripheral intravenous (PIV), 30 cases of PIV+ umbilical vein catheter (UVC), 43 cases of PIV+PICC, 136 cases of PIV+UVC+PICC. The average indwelling time of UVC was 6 days, the average indwelling time of PICC was 22 days. There were 23 cases (10.55%) got CRBSI and 195 cases (89.45%) without CRBSI. Binary Logistic regression analysis showed that birth weight ( OR=1.003, 95% CI 1.000-1.006, P<0.05) and combination form of venous access ( OR=0.139, 95% CI 0.023-0.834, P<0.05) of VLBW & ELBW infants were associated with CRBSI. Conclusions:In NICU, PIV, UVC and PICC are the three main ways to indwell venous access for VLBW & ELBW infants. The occurrence of CRBSI is closely related to the type and combination of indwelling venous access.

ABSTRACT

Objective:To investigate the effects of compound insulation measure in operation room on hypothermia of cesarean section of very low birth weight infant (VLBWI).Methods:A total of 96 VLBWI, who were born with cesarean section in the People`s Hospital of Guangxi Zhuang Autonomous Region from January 2018 to October 2020 and admitted to the neonatal intensive care unit were enrolled in the present study. They were assigned to observation group and control group according to the enrolled time, each group was 48 cases. The control group received routine protocol, the observation group implemented the compound insulation measures. The body temperature, heart rate, blood oxygen saturation (SpO 2), end tidal carbon dioxide partial pressure (P ETCO 2) were measured at birth, 10 min after birth and before entering the transfer incubator. The incidence of hypothermia, hypoglycemia and hypoxemia in premature infants were compared between the two groups. Results:At 10 min after birth and before entering the transfer incubator, the body temperature, SpO 2, P ETCO 2 were (36.58 ± 0.49) ℃, 0.95 ± 0.02, (37.17 ± 3.15) mmHg (1 mmHg =0.133 kPa) and (36.50 ± 0.55) ℃, 0.94 ± 0.02, (38.08 ± 3.85) mmHg in the observation group, which were significantly higher than those in the control group (36.27 ± 0.57) ℃, 0.93 ± 0.02, (35.85 ± 3.14) mmHg and (35.75 ± 0.48) ℃, 0.93 ± 0.01, (36.63 ± 3.17) mmHg, the differences were statistically significant ( t values were 2.03-7.13, all P<0.05). Before entering the transfer incubator, the heart rate was (140.25 ± 8.67) times/min in the observation group, which was significantly lower than that in the control group (145.89 ± 9.23) times/min, the difference was statistically significant ( t value was 3.09, P<0.05). The incidence of hypothermia, hypoglycemia and hypoxemia were 16.7%(8/48), 8.3%(4/48) and 12.5%(6/48) in the observation group, which were significantly lower than those in the control group 37.5%(18/48), 22.9%(11/48) and 29.2%(14/48), the differences were statistically significant ( χ2=5.28, 3.87, 4.04, all P<0.05). Conclusions:Evidence-based operation room compound insulation measure can effectively maintain the constant body temperature in VLBWI, reduce the incidence of hypothermia, hypoglycemia and hypoxemia.

OBJECTIVES@#To investigate whether evidence-based standardized nutrition protocol can facilitate the establishment of full enteral nutrition and its effect on short-term clinical outcomes in very preterm/very low birth weight infants.@*METHODS@#A retrospective analysis was performed on the medical data of 312 preterm infants with a gestational age of ≤32 weeks or a birth weight of <1 500 g. The standardized nutrition protocol for preterm infants was implemented in May 2020; 160 infants who were treated from May 1, 2019 to April 30, 2020 were enrolled as the control group, and 152 infants who were treated from June 1, 2020 to May 31, 2021 were enrolled as the test group. The two groups were compared in terms of the time to full enteral feeding, the time to the start of enteral feeding, duration of parenteral nutrition, the time to recovery to birth weight, the duration of central venous catheterization, and the incidence rates of common complications in preterm infants.@*RESULTS@#Compared with the control group, the test group had significantly shorter time to full enteral feeding, time to the start of enteral feeding, duration of parenteral nutrition, and duration of central venous catheterization and a significantly lower incidence rate of catheter-related bloodstream infection (P<0.05). There were no significant differences between the two groups in the mortality rate and the incidence rate of common complications in preterm infants including grade II-III necrotizing enterocolitis (P>0.05).@*CONCLUSIONS@#Implementation of the standardized nutrition protocol can facilitate the establishment of full enteral feeding, shorten the duration of parenteral nutrition, and reduce catheter-related bloodstream infection in very preterm/very low birth weight infants, without increasing the risk of necrotizing enterocolitis.

OBJECTIVES@#To investigate the changes in the rate and volume of mother's own milk (MOM) feeding for very low birth weight infants (VLBWIs) hospitalized during the prevention and control of the coronavirus disease 2019 (COVID-19) pandemic.@*METHODS@#A retrospective analysis was performed on the medical data of the VLBWIs with a gestational age of <32 weeks who were born and admitted to the Neonatal Intensive Care Unit of Nanjing Maternal and Child Health Hospital from January 2019 to December 2020. The changes in the rate and volume of MOM feeding for VLBWIs during hospitalization were examined.@*RESULTS@#A total of 301 VLBWIs were enrolled. According to the timing of COVID-19 outbreak, these infants were divided into a pre-CIVID-19 group with 205 VLBWIs and a post-COVID-19 group with 96 VLBWIs. Compared with the pre-CIVID-19 group, the post-COVID-19 group had a significantly lower rate of MOM feeding within 28 days after birth and during hospitalization (P<0.05), a significantly lower volume of MOM feeding within 0-7 days, 0-14 days, and 0-28 days after birth (P<0.05), and significantly higher incidence rates of moderate-to-severe bronchopulmonary dysplasia and feeding intolerance (P<0.05).@*CONCLUSIONS@#The COVID-19 pandemic has a significant impact on MOM feeding for VLBWIs, and there are significant reductions in the rate and volume of MOM feeding for VLBWIs within 28 days after birth, as well as a significant reduction in the rate of MOM feeding during hospitalization.

ABSTRACT

OBJECTIVES@#To study the effect of timing of surgical ligation of patent ductus arteriosus (PDA) on the prognosis of very low birth weight infants (VLBWI).@*METHODS@#The medical data of VLBWI who underwent transthoracic ligation for PDA from June 2018 to May 2021 were reviewed retrospectively. The infants were divided into early ligation group (≤21 days of age) and late ligation group (>21 days of age) based on the age of ligation. The two groups were compared in terms of perioperative clinical features, complications, and mortality. The risk factors for early surgical ligation were analyzed.@*RESULTS@#A total of 72 VLBWI were enrolled, with 19 infants (26%) in the early ligation group and 53 infants (74%) in the late ligation group. There were significant differences in birth weight, gestational age, weight at operation, days of age at operation, rates of preoperative invasive and noninvasive mechanical ventilation, incidence rate of pulmonary hemorrhage, incidence rate of hypotension, preoperative PDA internal diameter (mm/kg), intraoperative PDA external diameter (mm/kg), incidence rate of post-ligation cardiac syndrome, and duration of postoperative invasive mechanical ventilation between the two groups (P<0.05). A binary logistic regression analysis showed that pulmonary hemorrhage was an indication of early surgical ligation of PDA (P<0.05). There were no significant differences in the incidence rates of post-operative complications and the mortality rate between the early ligation and late ligation groups.@*CONCLUSIONS@#Early surgical ligation may be performed for VLBWI who are experiencing pulmonary hemorrhage and hemodynamically significant PDA confirmed by cardiac ultrasound after birth. However, post-ligation cardiac syndrome should attract enough attention. In addition, early surgical ligation of PDA does not increase the risk of surgery-related and long-term complications or death, indicating that it is a safe and feasible treatment option.

OBJECTIVES@#To study the influence of enteral feeding initiation time on intestinal flora and metabolites in very low birth weight (VLBW) infants.@*METHODS@#A total of 29 VLBW infants who were admitted to the Department of Neonatology, Children's Hospital of Chongqing Medical University, from June to December, 2020, were enrolled as subjects. According to the enteral feeding initiation time after birth, the infants were divided into two groups: <24 hours (n=15) and 24-72 hours (n=14). Fecal samples were collected at weeks 2 and 4 of hospitalization, and 16S rDNA high-throughput sequencing and gas chromatography-mass spectrometry were used to analyze the microflora and short-chain fatty acids (SCFAs) respectively in fecal samples.@*RESULTS@#The analysis of microflora showed that there was no significant difference between the two groups in Chao index (reflecting the abundance of microflora) and Shannon index (reflecting the diversity of microflora) at weeks 2 and 4 after birth (P>0.05). The analysis of flora composition showed that there was no significant difference in the main microflora at the phylum and genus levels between the two groups at weeks 2 and 4 after birth (P>0.05). The comparison of SCFAs between the two groups showed that the <24 hours group had a significantly higher level of propionic acid than the 24-72 hours group at week 4 (P<0.05), while there was no significant difference in the total amount of SCFAs and the content of the other SCFAs between the two groups (P>0.05).@*CONCLUSIONS@#Early enteral feeding has no influence on the diversity and abundance of intestinal flora in VLBW infants, but enteral feeding within 24 hours can increase the level of propionic acid, a metabolite of intestinal flora.

OBJECTIVES@#To study the value of bedside echocardiography in predicting persistent patency of the ductus arteriosus during the early postnatal period in very low birth weight (VLBW) infants.@*METHODS@#A retrospective analysis was performed for 51 VLBW infants who were admitted from March 2020 to June 2021, with an age of ≤3 days and a length of hospital stay of ≥14 days. According to the diameter of patent ductus arteriosus (PDA) on days 14 and 28 after birth, the infants were divided into three groups: large PDA group (PDA diameter ≥2 mm), small PDA group (PDA diameter <2 mm), and PDA closure group (PDA diameter =0 mm). The echocardiographic parameters measured at 72 hours after birth were compared among the three groups. The receiver operating characteristic (ROC) curve was used to evaluate the value of the echocardiographic parameters in predicting persistent patency of the ductus arteriosus (PDA≥2 mm) at the ages of 14 and 28 days.@*RESULTS@#On day 14 after birth, there were 17 infants in the large PDA group, 11 in the small PDA group, and 23 in the PDA closure group. On day 28 after birth, there were 14 infants in the large PDA group, 9 in the small PDA group, and 26 in the PDA closure group. There were significant differences in gestational age, birth weight, rate of pulmonary surfactant use, and incidence rate of hypotension among the three groups (P<0.05). PDA diameter, end-diastolic velocity of the left pulmonary artery, left ventricular output, and left ventricular output/superior vena cava flow ratio measured at 72 hours after birth were associated with persistent patency of the ductus arteriosus at the ages of 14 and 28 days (P<0.05), and the ratio of the left atrium to aorta diameter was associated with persistent patency of the ductus arteriosus at the age of 28 days (P<0.05). The ROC curve analysis showed that the area under the curve that the PDA diameter measured at 72 hours after birth predicting the persistent patency of the ductus arteriosus at the ages of 14 and 28 days was the largest (0.841 and 0.927 respectively), followed by end-diastolic velocity of the left pulmonary artery, with the area under the curve of 0.793 and 0.833 respectively.@*CONCLUSIONS@#The indicators obtained by beside echocardiography at 72 hours after birth, especially PDA diameter and end-diastolic velocity of the left pulmonary artery, can predict persistent patency of the ductus arteriosus at the ages of 14 and 28 days in VLBW infants, which provides a basis for the implementation of early targeted treatment strategy for PDA.

【Objective】 To analyze the risk factors of anemia in premature infants and the status of blood transfusion. 【Methods】 Clinical data of preterm infants with age ≤48 hours and hospitalization time ≥7 days, admitted to our hospital from August 2018 to July 2019, were collected. According to the Hb value, they were divided into the anemia group and the non-anemia group. The general information, disease occurrence, and treatment measures during hospitalization of the two groups were compared, and the risk factors affecting anemia were analyzed. 【Results】 A total of 169 cases were included in this study, including 88 cases in the anemia group and 81 cases in the non-anemia group. There were statistically significant differences between two groups in assisted reproduction 47(53.409) vs 33(40.741), basic Hb value 180.395(176.282, 184.508) vs 162.841(158.596, 167.085), maternal pregnancy anemia 27(30.682) vs 12(14.815), late-onset sepsis(LOS) 64(72.727) vs 31(38.272), mechanical ventilation 46(52.273) vs 10(12.346), neonatal respiratory distress syndrome(NRDS) 51(57.955) vs 26(32.099), coagulation dysfunction 27(30.682) vs 11(13.580) and average hospitalization days 31.276(26.885, 35.666) vs 15.798(14.251, 17.344), all P<0.05. Multivariate logistic results showed assisted reproduction, maternal pregnancy anemia, mechanical ventilation, coagulation dysfunction and average hospitalization days ≥20 were risk factors for preterm anemia. A total of 40 cases(23.669%) needed RBC transfusion to correct anemia during hospitalization, including 21 cases of very low birth weight infants(VLBWI). 8 patients(38.095%) received RBC transfusion ≥3 times during VLBWI hospitalization. The incidence of bronchopulmonary dysplasia(BPD) in the multiple transfusion group(≥3 times of RBC transfusion) was higher than in the non-multiple transfusion group. 【Conclusion】 Maternal anemia during pregnancy, assisted reproduction, mechanical ventilation, coagulation dysfunction and average hospitalization days are the related factors affecting the occurrence of anemia. The occurrence of BPD may be associated with multiple blood transfusions, but there is no clear evidence to prove that blood transfusion is an independent risk factor for the development of BPD, and a large sample, multi-center study is needed.

ABSTRACT

Objective:To understand the change trend and influencing factors of fecal calprotectin(FC) in very low birth weight(VLBW) infants, and to explore the application value of FC detection in the diagnosis of necrotizing enterocolitis(NEC) in VLBW infants.Methods:VLBW infants hospitalized in the neonatal department at Quanzhou Children′s Hospital from June 2018 to May 2019 were selected as research object for a prospective study.Fecal samples from the 1st, 7th, 14th, 21st, 28th and 35th days after birth and fecal samples from the acute and recovery stages of NEC were collected continuously.The content of FC was determined quantitatively by immunofluorescence assay.Results:(1) The FC level of non NEC VLBW infants from 1 to 35 days after birth was 143.5(47.8, 391.2) μg/g.Univariate analysis showed that the level of FC fluctuated with the postnatal age, the level of FC was the highest at 21 days, and then decreased.The level of FC increased significantly in formula feeding, premature rupture of membranes, neonatal sepsis, feeding intolerance and pregnant mothers without glucocorticoid before delivery( P< 0.05). (2) Multivariate covariance analysis showed that prenatal application of glucocorticoid( F=10.550, P=0.001), premature rupture of membranes( F=13.311, P<0.001), neonatal sepsis( F=8.001, P=0.005), feeding intolerance( F=4.751, P=0.030) and NEC( F=54.566, P<0.001) had significant effects on FC level.After controlling the effects of prenatal corticosteroid, premature rupture of membranes, neonatal sepsis and feeding intolerance, the levels of FC in NEC group and non-NEC group were 3 162.3(1 412.5-7 244.4)μg/g and 141.3(125.9-162.2)μg/g, respectively.In NEC group, the levels of FC in acute stage and recovery stage were 3 166.9(1 745.1, 6 806.4)μg/g and 130.9(97.4, 273.9)μg/g, respectively, with significant difference( t=10.304, P<0.001). While the levels of FC were 2 347.9(1 404.4, 5 893.4)μg/g in the mild NEC and 4 114.7(2 764.5, 9 208.4)μg/g in the moderate or severe NEC, respectively, with no significant difference( t=1.131, P=0.280). Conclusion:The levels of FC fluctuate with postnatal age and it is affected by multiple factors.FC maybe a useful marker for the diagnosis and evaluation of efficacy of NEC in VLBW infants.

OBJECTIVES@#To evaluate the accuracy and safety of measurements of transcutaneous carbon dioxide partial pressure (TcPCO@*METHODS@#A total of 45 very low birth weight infants were enrolled. TcPCO@*RESULTS@#There was no significant difference in TcPCO@*CONCLUSIONS@#Lower electrode temperatures (38-41℃) can accurately measure blood carbon dioxide partial pressure in very low birth weight infants, and thus can be used to replace the electrode temperature of 42°C. Transcutaneous measurements at the lower electrode temperatures may be helpful for understanding the changing trend of blood oxygen partial pressure.

OBJECTIVES@#To study the effect of the course of treatment with broad-spectrum antibiotics on intestinal flora and short-chain fatty acids (SCFAs) in feces of very low birth weight (VLBW) infants.@*METHODS@#A total of 29 VLBW infants who were admitted to the Neonatal Diagnosis and Treatment Center of Children's Hospital Affiliated to Chongqing Medical University from June to December 2020 were enrolled as subjects for this prospective study. According to the course of treatment with broad-spectrum antibiotics, they were divided into two groups: ≤7 days (@*RESULTS@#There was a significant reduction in Chao index of the intestinal flora in the ≤7 days group and the >7 days group from week 2 to week 4 (@*CONCLUSIONS@#The course of treatment with broad-spectrum antibiotics can affect the abundance, colonization, and evolution of intestinal flora and the content of their metabolites SCFAs in VLBW infants. The indication and treatment course for broad-spectrum antibiotics should be strictly controlled in clinical practice.

Resumen Introducción: La retinopatía del prematuro (ROP) es una de las principales causas de ceguera infantil. La inmadurez y la exposición a oxígeno son algunos factores de riesgo. El objetivo de este artículo fue analizar la frecuencia y los factores de riesgo de ROP en una cohorte de recién nacidos menores de 1,500 g. Métodos: Se llevó a cabo un estudio de casos (con ROP) y controles (sin ROP) de recién nacidos menores de 1,500 g. Se analizaron variables prenatales y neonatales, y para su comparación se utilizaron las pruebas estadísticas t de Student, χ2 y U de Mann-Whitney. Resultados: Se analizaron 282 recién nacidos: 152 (53.9%) con ROP y 130 (46.1%) sin ROP. La mayor frecuencia se encontró en los estadios 1 y 2, con 139 pacientes (91.4%), seguidos de los estadios 3 a 5, con 13 pacientes (8.5%). En los pacientes con ROP, el peso al nacer fue menor (902.7 vs. 1037.9 g; p < 0.0001), así como la edad gestacional (28.2 vs. 29.6 semanas de gestación; p < 0.0001). Los días de ventilación (32.8 vs. 16.1; p < 0.00001) y los días de oxígeno requerido durante la estancia hospitalaria (87.7 vs. 62.6; p < 0.0001) fueron mayores en los pacientes con ROP. La displasia broncopulmonar, la hemorragia intraventricular y la sepsis tardía fueron comorbilidades significativas para el desarrollo de ROP. Conclusiones: En este estudio, la frecuencia de ROP fue mayor que la reportada en la población mexicana, con una baja proporción de formas graves. La vigilancia estrecha del manejo de los neonatos con menor peso y menos edad gestacional es fundamental para lograr disminuir esta enfermedad.


Abstract Background: Retinopathy of prematurity (ROP) is the principal cause of blindness during childhood. The objective of this study was to analyze the frequency of ROP and risk factors associated with ROP in a cohort of very low birth weight infants. Methods: A cases (ROP) and controls (no ROP) study of infants less than 1500 g was conducted. Perinatal and neonatal variables were analyzed. For the statistical analysis, χ2 test, Student’s t-test and Mann-Whitney’s U-test were used. Results: For the study, 282 neonates were included: 152 (53.9%) with ROP and 130 (46.1%) without ROP. The most frequent stages observed were stage 1 and 2, with 139 (91.4%) patients, and stages 3 to 5, with only 13 patients (8.5%). In those neonates with ROP compared with neonates without ROP, the birth weight was less (902.7 vs. 1037.9 g) and the difference was significant (p < 0.0001). Also, the difference with gestational age (28.2 vs. 29.6; p < 0.0001), total ventilation days (32.8 vs. 16.1; p < 0.00001) and total oxygen days (87.7 vs. 62.6; p < 0.0001) was significant in neonates with ROP and neonates without the disease. Bronchopulmonary dysplasia, intraventricular hemorrhage and late onset sepsis were significant with patients with ROP. Conclusions: The frequency of ROP reported here is higher than the reported in Mexican population, with less cases of severe ROP. The neonatal surveillance in babies with less birth weight and gestational age is important to decrease the incidence of ROP.

Introducción. La anemia es una complicación para los recién nacidos de muy bajo peso al nacer, y los exámenes de laboratorio son un factor de riesgo preponderante. Más del 50 % recibe, al menos, una transfusión de glóbulos rojos. Estas se han asociado a mayor riesgo de infecciones, hemorragia intracraneal, enterocolitis necrotizante y displasia broncopulmonar. En 2012, se implementó, en el Hospital Italiano de Buenos Aires, una estrategia de menor volumen de extracción de sangre por flebotomía. El objetivo del presente estudio fue evaluar su asociación con el número detransfusiones.Métodos. Estudio cuasiexperimental del tipo antes/después. Se comparó el número de transfusiones entre dos grupos de prematuros de muy bajo peso con diferente volumen de extracción. Se evaluó la correlación entre el volumen extraído y el número de transfusiones estimando el coeficiente de Spearman. Para ajustar por confundidores, se realizó un modelo de regresión logística.Resultados. Se incluyeron en el estudio 178 pacientes con edad gestacional media de 29,4 semanas (desvío estándar: 2,7) y peso al nacer de 1145 gramos (875-1345). El perfil de la serie roja inicial fue similar entre ambos grupos. El número de transfusiones (p = 0,017) y el volumen transfundido (p = 0,048) disminuyeron significativamente. El coeficiente de correlación resultó de 0,83. En el análisis multivariado, volumen de extracción y peso al nacer se asociaron a un requerimiento mayor de 3 transfusiones.Conclusión. Un menor volumen de extracción de sangre en prematuros de muy bajo peso está asociado de manera independiente a menor requerimiento transfusional.


Introduction. Anemia is a complication in very low birth weight (VLBW) infants, and lab tests are a predominant risk factor. At least one red blood cell transfusion is given in more than 50 % of cases. Transfusions are associated with a higher risk for infections, intracranial hemorrhage, necrotizing enterocolitis, and bronchopulmonary dysplasia. In 2012, Hospital Italiano de Buenos Aires implemented a strategy to collect a lower blood volume by phlebotomy. The objective of this study was to assess its association with the number of transfusions.Methods. Before-and-after, quasi-experimental study. The number of transfusions was compared between two groups of VLBW preterm infants with different blood collection volumes. The correlation between the collection volume and the number of transfusions was assessed estimating Spearman's coefficient. A logistic regression model was used to adjust for confounders.Results. The study included 178 patients with a mean gestational age of 29.4 weeks (standard deviation: 2.7) and a birth weight of 1145 g (875-1345). The baseline red series profile was similar between both groups. The number of transfusions (p = 0.017) and the transfusion volume (p = 0.048) decreased significantly. The correlation coefficient was 0.83. In the multivariate analysis, collection volume and birth weight were associated with a requirement of more than three transfusions.Conclusion. A lower blood collection volume in VLBW preterm infants is independently associated with fewer transfusion requirements.

ABSTRACT

Objective@#To explore the study of synchronous disinfection of lower limbs and unilateral limb disinfection when peripheral inserted central catheters is inserted into the lower limbs of very low birth weight infants, and to find a better and safer method of puncture disinfection.@*Methods@#Total of 30 cases of children who received unilateral limb disinfection from January 2016 to February 2017 were selected as the control group, and 30 cases of children who received simultaneous disinfection of both lower limbs from March 2017 to June 2018 were selected as the experimental group, and non-random and non-synchronous control studies were conducted. The operation time, blood loss, times of placing sterile table and retained site of the two groups were analyzed retrospectively.@*Results@#The operation time of synchronous disinfection of lower limbs in the experimental group and the control group were (43.67±28.04) min, (60.33±35.40) min; the amount of blood loss were (1.64±0.90) ml, (2.34±1.50) ml, the differences were statistically significant (t values were 2.021, 2.176, P<0.05). In the control group, 76% (21/30) completed the PICC operation after laying a sterile operation table, while in the experimental group, 96% (28/30) did. In the control group, 5 cases were successfully indwelling via femoral vein, and in the experimental group, 2 cases were successfully indwelling via femoral vein.@*Conclusion@#The method of simultaneous disinfection of lower limbs increases the selection of peripheral veins, reduces the frequency of sterile table, blood loss and operation time, and reduces the number of inguinal vein puncture, which is relatively safe and effective.

Objective@#To investigate the effect of continuing nursing mode on mental state and the impact of care capacity in mothers of NICU children with very low birth weight.@*Methods@#Based on the theory of continuous care, a continuation nursing program for children with very low birth weight from the day of discharge from the hospital to one month after discharge was used. A randomized controlled trial was conducted on 80 cases of very low birth weight infants admitted to our hospital. The order of discharge was randomly divided into 40 cases in the intervention group and 40 cases in the control group. The control group received routine care and the intervention group used a continuing nursing intervention model. On the day of discharge from the hospital, 1 week after discharge, and 1 month after discharge, the scores of the mothers' psychological status and home care ability were collected.@*Results@#On the day of discharge, there was no significant difference in mental state and home care ability between the two groups (P>0.05). The anxiety, depression and negative coping scores of the intervention group were lower than the control group at 1 week and 1 month after discharge (1 week t value was 2.138, 2.068, 2.532, 1 mouth t value was 2.273, 3.564, 4.417, P<0.05) , and the scores and home care ability were actively coped. Higher than the control group (1week t value was -1.680, -2.970, P<0.05.1 month t value was -3.937, -7.156, P < 0.01). There was an interaction between intervention mode and intervention time 1 (F value was 8.46-64.38, P<0.01). There were significant differences between the two groups at different time and different groups (different time F value was 166.46-269.55, P<0.01; different groups F value was 5.26-13.55, P<0.05).@*Conclusion@#Through continuing nursing intervention for very low birth weight infants after discharge, it can effectively reduce the negative emotions of mothers and improve the positive coping style of mothers, then improve the confidence of caregivers and increase satisfaction so that we can provide a safe and harmonious family environment with premature babies after discharge.
